Coleen Nevin, MSN, RN, CMC, CHPN, is a Registered Nurse and Certified Care Manager with more than 30 years of experience across Acute Care, Home Health, Home Hospice, and Private Home Care. She began her career with Consultants-In-Care under the leadership of Vanessa Gang, RN, where she first worked as a Personal Care Aide while in Nursing School, and later as a Registered Nurse. Coleen went on to serve as a staff nurse at White Plains Hospital before joining the Visiting Nurse Service of New York’s Certified Home Health Agency, providing care to medically complex patients throughout Westchester and the Bronx. It was over these 10 years that she discovered her deep commitment to delivering skilled, compassionate nursing care in the home.

Influenced by both personal and professional experiences, Coleen was drawn to Hospice and Palliative Care. This led to almost 15 years with Calvary Hospital’s Home Hospice program as an RN Case Manager and later in clinical leadership roles. Her time at Calvary affirmed her passion for end-of-life care advocacy, education, and delivering dignified, person-centered support aligned with each individual’s values and goals.

Since 2023, Coleen has served as Director of Operations, Clinical & Quality Assurance at Compass Care Concierge Home Care, supporting older adults and their families in aging safely at home. Through this work, she recognized ongoing gaps in care navigation, communication, and support for individuals facing life-limiting illness.

This insight led to the creation of Solas Care Management & Consulting, a practice dedicated to guiding individuals and families through complex health decisions, care transitions, and end-of-life planning. Coleen provides compassionate care navigation, decision-support, and specialized education for healthcare professionals, care providers, and families—ensuring that no one has to face life’s most challenging moments alone.
